TWO PATIENT'S EXPERIENCE
Rhinoplasty: Functional and Reconstructive
Most plastic surgeons agree: rhinoplasty is the most challenging of
cosmetic procedures. A great deal goes into analysis and decision
making. In addition, healing forces in the nose can create uncertainty.
Yet the benefits of rhinoplasty can be tremendous. From an aesthetic
standpoint, the nose has a prominent position and is a defining feature.
In terms of function, ease of breathing is most affected by the narrow
regions just inside the nose—that is why all those pro athletes wear
strips to open this small, but critical area.
Techniques in rhinoplasty have never been better. Today’s surgeons
emphasize preservation of structure while improving looks. This more
conservative approach leads to noses that look good not just during the
first year, but for decades on. In addition, this new approach maintains
function, avoiding the great looking, tiny nose that the patient cannot
breathe through.
Dr. Hamilton is a surgeon who focuses on nasal surgery and truly
believes this specialization is critical to ensuring a successful outcome
for the patient. “Rhinoplasty is a procedure I perform weekly and I
think that makes an enormous difference. To do rhinoplasty well, you
really need to make it a focus of your practice.” Dr. Hamilton’s training
from residency through fellowship was always focused on nasal and
facial surgery. This depth of training combined with a practice that
focuses only on procedures involving the head, face, and neck makes
him uniquely qualified to perform rhinoplasty.
Recent Patient Experiences
Two recent patients of Dr. Hamilton’s demonstrate the range of
issues and goals associated with this procedure. One is a 20 year old
runner and National Guard member named Sarah who was
preparing for a tour in Iraq. Sarah had had severe nasal injuries as a
child that limited her breathing and distorted the appearance of her
nose. The other is Amy, a 21 year old student who is preparing to
move to New York and wanted to make a change in her nasal
appearance. Both of their stories give insight into the experience of
nasal surgery and highlight its benefits.

Sarah’s Case Study
Sarah came to Dr. Hamilton after being told correction of her nasal
fractures was too complicated by other surgeons. Her nasal fractures
were due to a series of injuries while a child. It severely affected her
breathing and from an aesthetic standpoint was noticeably distorted.
“Sarah’s nose may be one of the worst nasal
fractures I have seen” states Dr. Hamilton “the
septum (internal wall) of the nose” was fractured
at a 90 degree angle on itself and the outside—
well the pictures say it all.”
Sarah was told that improvement in her nose was
certainly possible, although a second procedure
down the road could be a possibility to achieve
full correction. Fortunately for Sarah most of her
issues were corrected in her first procedure.
Sarah’s surgery involved straightening both the
inside as well as the outside of the nose. It also
involved building up the structure of the lower
nose with cartilage from her ear to both give
support for breathing in this area as well as
to improve its definition and appearance.
Finally the first stage of revising a deep scar at
the base of Sarah’s nose was performed with
considerable improvement.
Recovery was much less involved than she
expected. By nine days out she could already
see a dramatic change in her appearance and
appreciate a new ability to breathe easily. With
a little make up from Michelle (see 9 days
image) Sarah was ready to get on with daily life.
Sarah is now three months out and can further
appreciate the changes in her appearance now
that most of the swelling has resolved. As with
most rhinoplasty patients, it will be a full year
before final results are realized.

Amy’s Case Study
Amy had a prominent nasal bump giving her a
profile she just wasn’t happy with, and had long
been self-conscious about the appearance of her
nose. She said, “I probably thought about it
since middle school.”
Amy’s procedure involved reducing both the
bone and cartilage that gave her such a strong
nose. In addition, techniques were used to
better define the base of the nose and upper lip
as well as the nasal tip. After surgery Amy was
surprised by how little pain there was. “There
was discomfort from stuffiness as you would
expect, but painful? Not really.”
Amy was back to work 10 days later and felt
great. Patients are required not to do anything
strenuous for two weeks and avoid contact
activities for 6 weeks. Most are like Amy,
however, and are back to regular activities much
quicker than they expect.
Amy is now one year out from her surgery and
still loves the new look of her nose—“I feel more
confident.” She is especially pleased with her
new profile.
Amy’s story is typical of many rhinoplasty
patients who think about doing something
about their nose for years, but once they make
the decision to have it done are thrilled with
the results.

People fromall walks of life elect to undergo nasal
surgery. While patients typically are younger
(patients should be at least 17 for women, 18 for
men), many patients because of time or financial
constraints wait until later in life to consult with
a surgeon. Dr. Hamilton says, “Many patients
present in their 50s or 60s and tell me they have
waited their whole life to have rhinoplasty.
These can be some of the most appreciative
patients and are really fun.”
